Fit notes & Sick notes

The Sick note also known officially as Fit note is a document that can be issued by a doctor to comment on your fitness to work. The regulations surrounding this is governed by the Department for Work and Pensions. More information on the fit note can be found here.

Please remember we can always back date sick notes (provided we have sufficient evidence of your illness at the time of request) but we cannot future date sick notes.

Counting Sick Days

When you are off from work you must count all calendar days that you have been off, so this includes any days you do not work, weekends or bank holidays.

Self Certify

if you have been off from work for seven days or less then you can self certify. This means you do not need a doctor to give you a sick note. To do this you can complete the online form and either print it or send it electronically to your employer. If you do not have access to a computer then you can pick up this form at the surgery. Simply ask the receptionists for a self-certificate form.

Sick Note (Fit Note) 

If you have been off from work for more than seven days then your employer is likely to ask you for a Sick note (Fit note) from your GP or hospital doctor. If you have been in hospital, then please request this from the hospital doctors. You can request a sick note by

  • Contacting us online
  • Using the NHS App
  • Calling the surgery

The sick note will either say "Not fit for work" or "May be fit for work". If it says "may be fit to work", you and your employer must agree on reasonable adjustments or changes to your work and duty. If you and your employer do not agree to the these adjustments and/or changes then you will be treated as not for work.

For your convenience we now text you, your completed sick note, which you can download securely to any device or computer. Therefore please make sure your mobile phone numbers are up to date when you make the request for the sick note.
